TEAR OFF CHECKLIST BEFORE MAILING Sample Submission Checklist Make sure that:Tubes are clearly labeled with sequential numbers, beginning with "1." There are no skipped or repeated numbers, tubes labeled with letters, or student names or initials. Samples DO NOT contain bromophenol blue loading dye - if it does, the sample set will not be processed The sealed zip-top plastic bag(s) include(s): tubes (DO NOT tape or parafilm the tubes in order to keep them shut) a photograph of the agarose gel the completely filled out Submission Form for each sample set Zip-top bag(s) are packed in a small Styrofoam box containing a cool pack. Ship by overnight mail to address below. There is no one to receive packages on the weekends. Do not send packages on Fridays.
